This Cookie Policy explains how Amae Health, Inc. and its subsidiaries and affiliates, including clinical practice entities (collectively, “Amae Health,” “we,” “us,” or “our”), use cookies and similar technologies on www.amaehealth.com and other websites of ours that link to this Cookie Policy (the “Sites”). It also explains the choices you have, including how to change your consent preferences at any time. This Cookie Policy supplements our Privacy Notice. It does not describe how we handle protected health information, which is covered by our HIPAA Notice of Privacy Practices.
1. WHAT ARE COOKIES AND SIMILAR TECHNOLOGIES?
Cookies are small text files placed on your device when you visit a website. They are widely used to make websites work, to remember your preferences, and to provide information to website operators. We also use similar technologies such as pixels, tags, scripts, and local storage, which we refer to collectively as “cookies” in this policy. Cookies set by us are “first-party cookies.” Cookies set by other companies whose tools we use are “third-party cookies.”
2. CATEGORIES OF COOKIES WE USE
We group the cookies used on the Sites into the following categories. The specific cookies in each category may change over time; the current list is available in the preference center described in Section 4.
Essential. These cookies are necessary for the Sites to function and cannot be switched off. They include cookies set by our website platform, our consent management tool (which records the choices you make), and the embedded referral form used by healthcare providers to contact us. These cookies do not track you across other websites.
Analytics. These cookies help us understand how visitors use the Sites so that we can improve them, for example which pages are visited most and whether visitors encounter errors. We use Google Analytics and Hotjar for this purpose. Hotjar may record how visitors interact with a page; we configure it so that the contents of form fields are never captured.
Advertising. These cookies are used to measure the effectiveness of our advertising and, where permitted, to show you relevant information about our services on other websites. We use Google Ads conversion measurement and may use the Meta (Facebook) Pixel for this purpose.
Embedded content. Some pages include content from third parties, such as Google Maps for clinic directions and YouTube for video. These providers may set their own cookies when the content loads. Where your consent is required, this content is held back until you enable it.
3. HOW WE OBTAIN YOUR CONSENT
When you first visit the Sites, a consent banner is displayed. What the banner asks of you depends on the privacy law that applies where you are located, which we determine from your IP address:
- California and other U.S. states with comprehensive consumer privacy laws. Analytics and Advertising cookies are not set until you choose “Accept.” You may also choose “Decline” or use “Preferences” to allow some categories but not others. A “Do Not Sell or Share My Personal Information” option is available.
- All other U.S. states. Analytics and Advertising cookies may be set when you visit, and the banner gives you the option to decline or adjust your preferences at any time.
- European Union, United Kingdom, and other regions. Non-essential cookies are not set until you opt in.
Essential cookies are set in all regions because the Sites cannot operate without them. Your choice is stored for 12 months, after which we will ask again.
4. HOW TO CHANGE YOUR PREFERENCES
You can review or change your choices at any time by selecting Consent Preferences in the footer of any page on the Sites. In California and other states with comprehensive privacy laws, the Do Not Sell or Share My Personal Information option is also available there. Withdrawing consent stops the related cookies from being set going forward; cookies already stored on your device can be removed through your browser settings.
Global Privacy Control. We recognize the Global Privacy Control (“GPC”) browser signal where required by applicable state law. If your browser or extension sends a GPC signal, we treat it as a request to opt out of the sale or sharing of your personal information and of targeted advertising for that browser, and our consent tool applies that preference automatically. Because GPC is a browser-level setting, you may need to enable it separately on each browser and device you use.
Browser controls. Most browsers let you refuse or delete cookies through their settings. Doing so may affect how the Sites work. Instructions for common browsers are available from their providers.
Opting out directly with providers. You can also opt out of Google Analytics using the Google Analytics opt-out browser add-on, manage Google advertising settings at adssettings.google.com, opt out of Hotjar at hotjar.com/policies/do-not-track, and manage Meta advertising preferences in your Facebook account settings.
5. HEALTH INFORMATION AND OUR FORMS
We do not use Analytics or Advertising cookies to collect the information you enter into our consultation request or referral forms, and session-recording tools are configured so that form contents are not captured. Information you submit through those forms is handled as described in our Privacy Notice and, where it is protected health information, our HIPAA Notice of Privacy Practices.
6. CHANGES TO THIS COOKIE POLICY
We may update this Cookie Policy from time to time to reflect changes in the cookies we use or in applicable law. The “Last Updated” date at the top indicates when it was most recently revised.
